Click here to submit a ticket and fill out the form to request a copy of the COA (Third-Party) Lab Tests.
As shown in the image below, select COAs and Lab Test from the "Type of Request" drop down menu, fill out all of the other fields, and click submit.
From there a ticket will be created in our support system, we will receive your request, and get back to you in 1-2 business days with the information requested.
